• Screening information about your eligibility, such as whether you meet the age requirement, your pregnancy and delivery history, height, weight and BMI, nicotine and THC use, medical conditions and medications, government assistance, housing and support system

Some of this is health information. We collect it only to evaluate whether you may qualify to become a surrogate, and you choose whether to provide it. We also record which page you submitted the form from.

Information collected automatically

When you visit this Website, we and our service providers automatically collect certain information, such as your IP address, browser and device type, the pages you view, the website or ad that referred you, and approximate location based on your IP address. Phone calls

4. Advertising, Analytics and Service Providers

Google Ads
We may use Google Ads conversion tracking to understand whether visitors who clicked one of our ads went on to submit an application. When an application is submitted, your email address and phone number may be shared with Google in a hashed (encoded) form so the submission can be matched to an ad click. Your screening answers are never sent to Google. You can manage ad personalization at https://adssettings.google.com.

CallRail
We use CallRail to display tracking phone numbers and to measure which pages and ads lead to phone calls. CallRail uses cookies to associate your visit with a call.

Formspree
Applications submitted on this Website are processed by Formspree, which delivers your submission to us.

Cloudflare
This Website is hosted and delivered by Cloudflare, which processes technical information such as IP addresses to deliver and secure the Website.
